Wellness Screenings
Wellness screenings include an individualized health risk assessment designed for early detection of health problems.
The wellness screening includes:
- On-line health risk questionnaire
- Blood cholesterol test
- Body fat analysis
- Blood pressure measurement
Results are made available to the patient in either a hard copy format or via email. After receipt of the results,
the patient can then make a follow-up appointment to discuss the results and identify specific goals with a
professional member of the SWC staff.
We emphasize confidentiality and patient privacy throughout the testing and interpretation sessions. The wellness
screening is available by appointment.
